School Physicals

Dear Parent/Guardian of an Incoming Seventh Grader

In accordance with the Massachusetts State Law [105 CMR 200.400-200.920], all school children in grade 7 are required to undergo a physical examination upon entrance to 7th grade. In order to comply with health insurance coverage, an acceptable physical would be from February 1, 2010 to February 28, 2011 (a one year period of time). This may mean that you submit a copy of immunizations in Sept. and a copy of a recent physical some time later.

ALL STUDENTS ENTERING 7TH GRADE, REGARDLESS OF PHYSICAL DATE, MUST SUBMIT VERIFICATION OF A TETANUS BOOSTER, 2 DOSES OF VARICELLA VACCINE AND MUST BE IN COMPLIANCE WITH THE STATE'S CURRENT IMMUNIZATION LAWS AND REGULATIONS. IF VERIFICATION OF THESE REQUIREMENTS ARE NOT RECEIVED BY SEPTEMBER 24, 2010, YOUR CHILD WILL NOT BE ALLOWED TO ENTER SCHOOL. ANY NON-COMPLIANT STUDENT WILL BE TURNED AWAY AT THE DOOR BY SCHOOL ADMINISTRATION.

PLEASE BE SURE TO DELIVER ANY DOCUMENTATION TO NURSE'S OFFICE. I DO NOT RECEIVE COPIES OF THOSE GIVEN TO SPORTS COACHES

This letter is provided to you, now, more than 6 months in advance, to allow adequate time to ensure that your child will be able to enter school in September, 2010.
