Mandatory Health Requirements for Entrance into 7th Grade

1.) Update copy of Immunizations must be submitted prior to first day of school.

You must submit evidence of these immunizations prior to start of 7th Grade or your child may be turned away from school. This is STATE LAW MANDATE. REGARDLESS OF YOUR YEARLY PHYSICAL DATE, YOU CANNOT BE DENIED AN IMMUNIZATION APPOINTMENT FROM YOUR PEDIATRICIAN, TO RECEIVE NECESSARY IMMUNIZATIONS.

2.) Copy of Recent Physical.
ACCEPTABLE DATES ARE 2/1/11-2/28/12. Please submit a copy to the Nurse's Office as soon as Physical is complete. Copies sent to Athletic coaches are not received by this office.
