Parent/ legal Guardian must accompany student during registration to provide accurate information
    and to sign necessary forms. Custody papers must be provided if they apply.

Student must have a transcript from the last school attended in order to be assigned proper grade level.

Students must have sign out grades or grades to date.

Official transfer card or letter of withdrawal from previous school.

MCAS results for students transferring from a Massachusetts school. MCAS results are needed for ELA, math and
     science.

A discipline and attendance report from previous school must be provided. If the child did not have any
    discipline issues, a statement from the school on their letterhead is sufficient.

A copy of the student's birth certificate.

Proof of Residency (utility bill, mortgage/rent receipt, etc.) and a drivers license/ID with Saugus address.

Immunization Record which will be reviewed by our School Nurse.

Students who require special education support must have an IEP/504 at time of registration. This information must
     be provided prior to the day of registration to ensure special education staff is available to review the IEP/504.

Students, whose home language is that other than English, are required to be language tested. This
     information must be provided prior to the day of registration to ensure that staff is available.

Student will start school the morning after registration is completed.
